July 1, 2007 - June 30, 2008 QA Indicators:

All 12-Lead EKG usage-including the separate QA Form (NOTICE: This is the MEMS 12 Lead QA Form) This form requires Acrobat Reader

 

All Advanced Airway procedures: i.e. OPA, NPA, ET, LMA, Combitube, Cricothyrotomy, including MEMS QA Form. This form requires Acrobat Reader

 

QA Officers: if you do not have any runs that meet the indicators, please notify me of that, thank you.

Remember if your service is more than three (3) months behind in QA, the Regional Medical Director will not sign your license application.
