All Advanced Airway procedures (ET, King, LMA, Combitube, Cricothyrotomy) Airway QA Form

All CPAP utilization

QA Officers: if you do not have any runs that meet the indicators, please notify the office.

Remember if your service is more than three (3) months behind in QA, the Regional Medical

Director will not sign your license application.
